The app is a personal recordkeeping and appointment-preparation tool. It displays information entered by the user or imported from third-party sources. Those readings, transcriptions, dates, attachments, summaries, and reports can be incomplete, delayed, duplicated, mistranscribed, or inaccurate.

Do not delay care

Do not use Health Trailbook to decide whether an emergency exists or to replace a qualified healthcare professional. Call 911 or the appropriate local emergency number for trouble breathing, chest pain, stroke signs, severe allergic reactions, loss of consciousness, or other urgent symptoms.

Verify before sharing

Review every entry and report. Confirm device readings against the original device or report, and discuss questions with a qualified healthcare professional. Health Connect, Kardia, OMRON, VeSync, Beyond Oximeter, speech recognition, and other third-party products are independent services and are not controlled or certified by CPW Studio Co.

No clinician relationship

Use of Health Trailbook does not create a doctor-patient, nurse-patient, pharmacist-patient, or other professional healthcare relationship with CPW Studio Co.
